How do I add a multi-choice question to my survey where the employee can select as many answers as they want that apply to the question?


To add a check box multi-choice question where the employee can select as many answers that apply to your survey select “Create Survey,” and after completing the survey Setup questions select “Survey Builder” from the left hand side menu. Click the drop down arrow on the ‘Select the type of question” box and select ‘Check box” and click on the green “Add Question” button. Type in your question in the question box and any description you wish to appear underneath the question in the description box. Add the different optional answers that the employee can select from under “Values”. You can choose whether the employee must answer this question or not by using the “Is required” slider. When complete click the green “Add” button.
